Osteotransductief calcium-fosfaat (Ca-P) cement (NGN.4484)

Omschrijving van het onderzoek

In orthopaedic surgery the succes of an implantation depends on the quality of the fixation of the implant in the bone. A commonly used method of fixation method is the one that uses polymethylmethacrylate (PMMA) as bone cement. However, it has already been known for some time that orthopeadic implants with PMMA can give problems at a later stage.
In dental practice there is the need to have a material that not only prevents resorption of the bone after the loss of normal teeth, but which would also allow to place a dental implant immediately after extraction.
These problems can all be overcome by the use of kneadable calcium phosphate cements, which are biologically compatible with bone, and which will slowly be resorbed and replaced by natural bone.
The aim of this study is the further development an optimization of CaP-cements as a clinically safe product, that can be used for the fixation of bone implants and restoration (filling) of bone defects.

Resultaten van het onderzoek

In a first series of studies, the clinical handling and biological properties of two new Ca-P cement formula's were evaluated. Therefore, animal experiments (goats) were done. The new materials were placed subcutaneous as well as into the femoral condyle and tibia of the animals. Clinically, the material appeared to fullfill all the expectations. The cement was easily injectable, could be shaped after injection and did set within a proper time limit (even when in contact with blood). Further, histology revealed that the bone biocompatibility of the cement was excellent. Already, within two weeks after implanttaion, the material became completely covered with bone. On the other hand, the material showed only very limited signs of degradation. The above mentioned findings make that in follow-up investigations, two different routes will be pursued:

  1. we will try to improve the degradation properties of the cement.
  2. we will test the suitability of the Ca-P cement for the fixation of dental and orthopaedic implants. However, in this research we will change the cement-concept , as currently used for PMMA-cement. The Ca-P cement will only be applied as a very thin layer between implant and drill wall. The cement layer only has to fill all the present gaps and voids in order to increase bone bridging. The reason for this modified concept is that the Ca-P cement remains a ceramic. Because of the current very slow degradation behavior, a thick cement mantle can not withstand the applied forces during loading and will fracture.
